Docs: choosing a plugin placement, and the gates for public endpoints (#133) - #140

Add "Choosing a placement" and "Public endpoints need a gated server" to integrations/README.md, summarising sections 0 and 1 of the accepted #88 design (docs/design/server-plugin-routes.md): placements A-E with their decision rules, and the three gates (Cargo feature `plugin-routes`, `--plugin-routes`/`ATOMIC_PLUGIN_ROUTES`, install consent). Everything not built yet is marked planned with its ontola/atomic-server issue (#1711-#1723); atomic.place builds without the feature. Link it from AGENTS.md's "Plugin runtimes".
